The Senior Case Manager will be responsible for building upon Lost Angels' existing case management work and to develop an entry-level, employment readiness program to include barrier reduction, supportive and wraparound services, job readiness, and case management for participants. The Senior Case Manager will develop, manage, and oversee the intake, servicing, and case management supportive services of all clients. This position will ensure that clients are assessed for entry-level employment readiness and work, and manage and track all supportive services rendered.

Roles & responsibilities

Responsible for developing, managing, and overseeing a timely, accurate and complete intake process for expanded entry-level employment readiness case management program.

Extensive knowledge and awareness of available programs and services throughout the community; and the ability to navigate systems, develop and maintain relationships with key collaborative partners.

Ability and skill set to create an individual and detailed plan to support and reduce barriers for clients – this will include, but not limited to, education services and planning, and securing necessary personal documents (i.e., social security cards, birth certificate, identification, driver’s license, etc.).

Assessment of individual client needs on a case-to-case basis.

Understanding of key organizations, DPSS, to aid in the successful implementation of services and assist clients in accessing all needed services (i.e., client enrollment, application, etc.).

Commitment to assisting youth with barrier reduction strategies and milestones on the individual development plans – this can include collaborative meetings with educational partner, tutor, etc.).

Managing data on the CRM to provide complete documentation of all contacts, barrier education, updates, and any other pertinent information on each client, ensuring data entry will be done consistently, accurately, and effectively on a daily and weekly basis.

Complete weekly and monthly reports on client progress, barrier reduction, attendance, and key performance indicators and demographic reports and outcomes report, as requested and needed.

Ability to be empathetic and offer support and compassion to client struggles and barriers, while offering solutions and strategies for success.

Ability to develop strong and successful relationships with partners focused on the client target population and services tailored to our clients.

Strong administrative skills, this includes experience with Microsoft Office, Word, Excel, PowerPoint, resume development and our internal CRM.

Commitment to regular servicing and auditing of client files for quality assurance and quality improvement efforts.

Qualifications

7-10 years case management experience

Preferred:

Lived experience

4-year college degree
