    NHF's Building Horizons: Improving Social Connectivity Across Generations

    National Health Foundation’s Bridging Horizons program engages transitional-aged youth (TAY) and older adults in intergenerational dialogue and activities to reduce social isolation and improve community health outcomes. Through structured mentorship, wellness workshops, and community meetings, the program fosters meaningful connections and enhances participants' mental well-being. This initiative will utilize a comprehensive evaluation process to track progress and measure community level improvements in social connectivity.

    National Health Foundation Arleta Recuperative Care Program

    There are 15,000 older adults sleeping unsheltered on the streets of Los Angeles. This number is expected to triple by 2030. National Health Foundation is opening a location in Arleta CA that will provide a new type of tailored care management and housing option for older adults (55+). This 43,000 square foot facility will house 148 older adults experiencing homelessness until they are connected to permanent housing or other forms of supportive housing. This care model will include 74, 100% ADA compliant, semi-private bedrooms and facilities.

    An Innovative Model for Caring for Our Aging Homeless

    NHF is launching an innovative148 bed interim housing facility that will be uniquely designed for older adults (ages 55+) who have experienced homelessness. Through its trauma-informed model of care, NHF and its partners will provide medical support and social services for its guests that are built on a foundation of trust and care, with the goal of transitioning our neighbors into permanent housing. The facility will also serve as a hub for community engagement, providing social services and medical support to low-income residents in the area.

    Youth Organizing for Better Health in Pico-Union

    National Health Foundation is building a youth organizing base in the Pico-Union neighborhood of Los Angeles. In partnership with West Adams Preparatory High School, NHF will engage students in researching barriers to healthy lifestyles in their community and implementing real changes that have lasting impacts. Woodbury University will support this project by developing a mobile application with an interactive map/database of community resources that youth identify through their research.
